Access to investigational drugs is a concern to patients and regulatory agencies. In order to determine potential trial participants’ views on access to investigational drugs, we surveyed one hundred people who had been referred to a phase I clinical trial. Most respondents indicated that patients had a right to investigational drugs, that the drugs should be offered only in the context of research, that getting access to these drugs is too hard, and that knowing the right people and being persistent (...) increased the likelihood of gaining access. Respondents did not think that investigational drugs should be given to anyone who wanted them, or that physicians were aware of the latest investigational agents. They most frequently recommended two allocation criteria: offering investigational drugs to those who would benefit most or were most needy; and allocating them to maximize scientific advancement. Respondents who understood the purpose of the trial were more likely to choose the second criterion. (shrink)

The Enlightenment has often been written about as a sequence of disembodied 'great ideas'. The aim of this book is to put the beliefs of the Enlightenment firmly into their social context, by revealing the national soils in which they were rooted and the specific purposes for which they were used. It brings out the regional divergences of the Enlightenment experience, shaped by different local intellectual and economic priorities. At the same time it also shows how central concerns were shared (...) everywhere, and how the writings of certain key areas came to be influential elsewhere. The thirteen essays, each written by a historian specialising in the particular country, examine national contexts from Sweden to Italy, from Russia to North America. Multidrug resistant organisms are a public health threat that have reduced the effectiveness of many available antibiotics. Antimicrobial stewardship programs have been tasked with reducing antibiotic use and therefore the emergence of MDROs. While fecal microbiota transplant has been proposed as therapy to reduce patient colonization of MDROs, this will require additional evidence to support an expansion of the current clinical indication for FMT. It is demonstrated that a non Abelian Stokes Theorem is necessary to describe the B3 field of radiation. A simple form of the theorem is build up from the fundamental definition of B3 in O(3) gauge field theory, which is a gauge field theory applied to electrodynamics with an O(3) internal gauge symmetry bases on a complex basis ((1), (2), (3)). The indices (1) and (2) are complex conjugate pairs based on circular polarization, and the index (3) is aligned with (...) the propagation axis of radiation. (shrink)

A popular goal in psychological science is to understand human cognition and behavior in the ‘real world’. In contrast, researchers have typically conducted their research in experimental research settings, a.k.a. the ‘psychologist’s laboratory’. Critics have often questioned whether psychology’s laboratory experiments permit generalizable results. This is known as the ‘real-world or the lab’-dilemma. To bridge the gap between lab and life, the concept of ecological validity has been widely used to evaluate whether laboratory experiments resemble and generalize to the ‘real (...) world’. However, researchers seldom define what they mean with this concept, nor are there any guidelines or criteria available for evaluating a study’s ecological validity. In our view, the popular concept of ecological validity is ill-formed, lacks specificity, and falls short of adequately addressing the fundamental problem of generalizability. In order to move beyond this ‘real-world or the lab’-dilemma, we believe that researchers should be more specific about the contexts of behaviors they are interested in, instead of aiming to understand human cognition and behavior in the ‘real world’. We argue that this is a more constructive way of uncovering the context-specific and context-generic principles of human cognition and behavior. (shrink)

Using covariant derivatives and the operator definitions of quantum mechanics, gauge invariant Proca and Lehnert equations are derived and the Lorenz condition is eliminated in U(1) invariant electrodynamics. It is shown that the structure of the gauge invariant Lehnert equation is the same in an O(3) invariant theory of electrodynamics.

We disagree with Almaatouq et al. that no realistic alternative exists to the “one-at-a-time” paradigm. Seventy years ago, Egon Brunswik introduced representative design, which offers a clear path to commensurability and generality. Almaatouq et al.'s integrative design cannot guarantee the external validity and generalizability of results which is sorely needed, while representative design tackles the problem head on.

Nanoparticles of cobalt ferrite prepared by the co-precipitation method with crystallite size varying from 4.7 to 41 nm have been characterized by positron annihilation lifetime spectroscopy. Three lifetime components are fitted to the lifetime data. The shortest lifetime component is attributed to the delocalized positron lifetime shortened by defect trapping. The intermediate lifetime is assigned to the positron annihilation in diffuse vacancy clusters or microvoids at the grain boundaries and at the grain-boundary triple points. The longest component corresponds to the (...) pick-off annihilation of ortho-positronium formed at the larger voids. The variations in these lifetimes and their relative intensities with annealing temperature and crystallite size have been studied in detail. (shrink)

Our society has long sanctioned, at least tacitly, a degree of conflict of interest in medical practice and clinical research as an unavoidable consequence of the different interests of the physician or clinical investigator, the patient or clinical research subject, third party payers or research sponsors, the government, and society as a whole, to name a few. In the past, resolution of these conflicts has been left to the conscience of the individual physician or clinical investigator and to professional organizations. (...) The public is no longer willing to allow health care providers to wholly govern their own conflicts of interest for several reasons. These include: new forms of health care financing and delivery that provide innovative and lucrative opportunities for physician or insurer enrichment at patient expense; the increased importance of commercial research support as peer-reviewed governmental research support has decreased; evidence that physicians and clinical investigators too frequently resolve conflicts of interest in their own favor; and a general societal mistrust of authority. This volume represents a multidisciplinary effort, drawing from philosophy, medicine, law, economics and public policy to identify and categorize conflicts of interest in medical practice and clinical research, and, where possible, to offer a mechanism for resolving them. Part I addresses conflicts of interest from a theoretical perspective, offering basic concepts and analytical frameworks. The second part discusses two topics prominent in current health care policy debates--self-referral and financial incentives to limit care. Part III examines conflicts of interest generated by pharmaceutical industry involvement in clinical practice and research. The final section deals with conflicts of interest in clinical research in several contexts, including institutional reviews boards, clinical trials, Cooperative Research and Development Agreements between government and private researchers, brokerage of research subjects by Contract Research Organizations, and cost-effectiveness studies. The paper argues against Peacocke's moderate rationalism in modality. In the first part, I show, by identifying an argumentative gap in its epistemology, that Peacocke's account has not met the Integration Challenge. I then argue that we should modify the account's metaphysics of modal concepts in order to avoid implausible consequences with regards to their possession conditions. This modification generates no extra explanatory gap. Yet, once the minimal modification that avoids those implausible consequences is made, the resulting account cannot support (...) Melanie presented at twenty weeks of gestation to an obstetrics clinic in a critical access hospital in rural Vermont. She was excited to undergo routine fetal ultrasonography, but her obstetrician gave her grave news: the ultrasound revealed hypoplastic left heart syndrome, a devastating congenital heart defect. Initially, Melanie agreed in general to pursue surgical care for her fetus—a three‐stage process that has somewhat uncertain results and could only be done in tertiary care facilities far from her home in Vermont. A (...) week later, while the maternal fetal medicine and pediatric cardiology units made arrangements with colleagues in Boston, Melanie began having second thoughts. Emotions play a prominent role in social life, yet the direct impact of emotions on behavior and judgment remains a point of disagreement. The current investigation used meta-analysis to test two theoretical perspectives. The emotion-as-direct-causation perspective asserts that current emotions guide behavior and judgment, whereas the emotion-as-feedback perspective asserts that anticipated emotions guide behavior and judgment. Although the emotion-as-direct-causation perspective was frequently tested, only 22% of tests were significant. Although the emotion-as-feedback perspective was rarely tested, 87% of tests were significant. (...) Our findings suggest that empirical evidence is weak for the default assumption that emotion is the proximal cause of behavior and judgment. Our preliminary findings also suggest that anticipated emotion reliably impacts social behavior and judgment. (shrink)

The paper compares the suitability of two different epistemologies of counterfactuals—(EC) and (W)—to elucidate modal knowledge. I argue that, while both of them explain the data on our knowledge of counterfactuals, only (W)—Williamson’s epistemology—is compatible with all counterpossibles being true. This is something on which Williamson’s counterfactual-based account of modal knowledge relies. A first problem is, therefore, that, in the absence of further, disambiguating data, Williamson’s choice of (W) is objectionably biased. A second, deeper problem is that (W) cannot satisfactorily (...) elucidate modal knowledge. Third, from a naturalistic perspective, the nature of this second problem favours (EC) against (W). Following on from Roy Bhaskarâe(tm)s first two books, A Realist Theory of Science and The Possibility of Naturalism, Scientific Realism and Human Emancipation, establishes the conception of social science as explanatoryâe"and thence emancipatoryâe"critique. Scientific Realism and Human Emancipation starts from an assessment of the impasse of contemporary accounts of science as stemming from an incomplete critique of positivism. It then proceeds to a systematic exposition of scientific realism in the form of transcendental realism, highlighting a conception of science as explanatory (...) of a structured, differentiated and changing world. Turning to the social domain, the book argues for a view of the social order as conditioned by, and emergent from, nature. Advocating a critical naturalism, the author shows how the transformational model of social activity together with the conception of social science as explanatory critique which it entails, resolves the divisions and dualisms besetting orthodox social and normative theory: between society and the individual, structure and agency, meaning and behavior, mind and body, reason and cause, fact and value, and theory and practice. The book then goes on to discuss the emancipatory implications of social science and sketches the nature of the depth investigation characteristically entailed. In the highly innovative third part of the book Roy Bhaskar completes his critique of positivism by developing a theory of philosophical discourse and ideology, on the basis of the transcendental realism and critical naturalism already developed, showing how positivism functions as a restrictive ideology of and for science and other social practices. Shankara was born in the eighth century on the west coast of south India. After devoting himself to yoga practices and meditation, Shankara wrote commentaries on the Bhagavad Gita, some of the Upanishads and other scriptures, and travelled throughout India declaring the oneness of a supreme reality and refuting erroneous philosophical doctrines. He reorganized the ancient, renunciate swami order and established permanent monastic centres in four regions of India: Sringeri in the south, Puri in the east, Dwaraka in the west, (...) and Badrinath in the Himalayas. Shankara lived during an era when Sanatana Dharma was beginning to be more widely emphasized, authoritative leadership was lacking, and conflicting religious sects were promoting a variety of philosophical opinions. His emphasis on the oneness of a supreme reality and the divinity of each person was a harmonizing influence. Besides expounding his non-dualistic views as presented in Self-Knowledge, he also wrote poems and composed hymns to reverently honour a variety of gods and goddesses to inspire and encourage people in all walks of life to be devoted to right living and spiritual practices.
